To fix this issue:
Go to the location C:\UBSACC2015\ and look for the data folder like C:\UBSACC2015\access_software_sdb_bhd\
Look for files ending with .mem and delete all of them.
Open UBS, and it will prompt to upgrade data files.
You will be able to log in and ensure to maintain the housekeeping setup again for company details, as this was reset to default.
