To fix this issue, you need to set up the EIS Addresses & Account No.:
Open UBS Payroll (Modern View).
Go to Settings, Bank Settings, Addresses & Account No.
Click View List.
Choose number 28.
Go to Your Company Registered Info tab.
Key in the Registered Name, File No.
Choose (if any) the APS Format No. In Aps_Set.DBF.
Click Save.
Go to Employees.
Choose the respective employee.
Click Edit Employee under Actions.
Go to the Statutory Info tab.
Under EIS info, choose the Category using the drop-down button.
Click Save.
Go to Reports.
Under Statutory Reports, choose EIS (Employment Insurance System).
Under EIS Lampiran 1, choose respective File No.
Proceed to Generate Excel.
Note: If most employees appear in the EIS Lampiran 1 report but only one or two are missing, check whether the EIS Category is correctly set under Personal File Maintenance for those employees.
